This series was recorded at "Critical Conversations: An Advance Care Planning Summit for Advocates" on October 11, 2018. The goal of these recordings is to provide you with the tools to engage in important, and sometimes challenging, advance care planning conversations. The knowledgeable speakers included in this series of recordings will offer inspiration and information to help you assist patients, clients, and families with thinking and talking about their end-of-life health care preferences.

Since 2014, the East Bay Conversation Project has promoted advance care planning in Alameda and Contra Costa counties. Through the efforts of our two steering committees, quarterly training workshops, presentations, community events, and summits, we’ve outreached to more than 4,000 individuals and trained more than 375 advance care planning advocates about the importance of having “The Conversation” to help ensure patients and families experience better end-of-life care.     End-of-Life Options: Navigating Difficult Healthcare Discussions

    This interactive and informative session will provide an overview of end-oflife options including how to answer questions about voluntary stopping of eating and drinking (VSED), palliative sedation, and the California End-ofLife Option Act law and status.  We will explore how to address these end-oflife options and how they interplay in advance care planning.  Tools and resources will be shared to empower and encourage better end-of-life conversations that focus on patient-centered care.

    Conversation Starters: Tools and Strategies

    In this interactive session, participants will work directly with specific tools and strategies to enhance their ability to guide advance care planning conversations with patients, clients, and families. Topics will include: overcoming discussion barriers, negotiating conflict, helping people understand aspects of an effective health care agent, and facilitating consideration of values to help ensure people receive the care they want.

    Not Just the Forms: Ethical and Legal Dimensions of the Conversation

    Health care providers are obligated to honor patient wishes and preferences as documented in advance care planning forms. These forms are an essential tool in providing coordinated care across multiple care settings. However, challenges can arise when there is uncertainty around the interpretation and application of these forms. This presentation will discuss the purpose and function of the forms and the ethical and legal considerations of “The Conversation” when discussing the patient’s preferences and wishes.

    Living with Dementia: Honoring Wishes 

    Rachel Main, Alzheimer’s Association, Northern CA and Northern NV 

    Discussing and planning for end-of-life care isn't always easy, especially if you have received a diagnosis of dementia.  This session will provide an overview of advanced and end-stage dementia and its unique considerations, a summary of results and recommendations from a San Francisco survey of health care providers and caregivers, a review of dementia specific end-of-life planning resources, and a discussion of the importance of why, when, and how to discuss end-of-life issues with people diagnosed with dementia and their care partners.

    Cultural Considerations: A Panel Discussion

    Advance care planning conversations benefit from including awareness of and sensitivity to different cultural practices and beliefs of patients and families. Communicating effectively to increase our understanding of individual, cultural, and spiritual approaches to health care choices is an important component of effective advance care planning. Learn how to view end-of-life care through a cultural lens. This general session panel will share important insight into the perspectives of the Chinese, African American, Latino, and LGBTQ communities when engaging in discussions about endof-life care preferences.
