NOTE: This event will take place at John Muir Medical Center - Concord Campus located at 2540 East Street, Room Concord I & II, Concord, CA 94520.

Advance care planning enhances quality of life by ensuring that plans are developed to address end-of life care decisions in accordance with personal wishes. Join Janet Thompson, senior living counselor at St. Paul’s Towers, to learn more about methods to engage others in this important conversation and to teach it effectively          

After attending this training, attendees will be able to:                                            

  • Identify the concepts and benefits of advance care planning.
  • Utilize the tools available to engage in discussion of advance care planning
  • Distinguish the importance and characteristics of an effective Health Care Agent
  • Complete and assist others with advance health care directives and POLST forms

CE credits are FREE to everyone for this event / 3.75 CME credits are free to ACCMA members ($187.50 for non-members)

Janet Thompson

Senior Living Counselor

Janet Thompson has been at Mercy Retirement & Care Center for 9 years as Sales & Marketing Director where she has asked many family members to provide advance care plans. She is an experienced, creative and insightful leader with a passion for working in the geriatric care field. As guardian for her mentally challenged uncle and POA for her parents, all who deceased at young ages from significant health issues, Janet believes strongly in advance care planning as a natural part of one’s health coordination.

Janet has a BA in Communications from the University of Michigan and an MBA-Executive Program from St. Mary’s College. She is certified by the Coalition of Compassionate Care of California, and is a part of several associations including the California Assisted Living Association (CALA), LeadingAge, and Partners In Care Alameda. Janet is dedicated to the advance care planning and POLST movement and helping people move from “I’ll make my plan some day in the future” to “I’ll finish my advance care plan by next week.”
