Discussing and planning for end-of-life care isn't always easy, especially if you have received a diagnosis of dementia.  This session will provide an overview of advanced and end-stage dementia and its unique considerations, a summary of results and recommendations from a San Francisco survey of health care providers and caregivers, a review of dementia specific end-of-life planning resources, and a discussion of the importance of why, when, and how to discuss end-of-life issues with people diagnosed with dementia and their care partners.
