Since 2009, the California POLST form has helped give patients a voice to communicate their end-of-life health

care wishes. Learn how you and your organization can provide more eective POLST conversations this summer

through this one-hour webinar, presented by the Coalition for Compassionate Care of California (CCCC) and

the Alameda-Contra Costa Medical Association (ACCMA).

*Non-ACCMA members who wish to claim CME credits for this program will be charged a $50 admin fee to help cover the costs of providing CME.
